Dundee Learning Disability Service

Turning Point Scotland in Dundee supports people who have a learning disability to live in their own homes.

The service works in partnership with Dundee City Council and Angus Council social work departments to provide support to ten people in six houses in the Dundee area.

The houses are provided by Hillcrest Housing Association and we work with them to support people to maintain their tenancies. The houses are based centrally in Dundee enabling easy access to facilities within the local community.

Each service user has a key worker who works with them and other significant people (families, advocacy, day care services).  Person centred support plans are developed specific to each individual; these provide fine detail into the appropriate support that is provided for each person. 

Through support plans service users will be supported in areas such as:

personal care skills
health (physical and mental wellbeing)
medication
promoting appropriate communication
day support services including leisure activities
relationships and family contact
financial support

Support to service users is provided by three staff teams working across the service.  The support provided ranges from a few hours a day to full twenty-four hour support with a sleepover.  The amount of support provided varies depending on the needs of the individual and is agreed with Care Management teams.

Additionally we work with Capability Scotland, who provides an enabler service. Sense Scotland provides day care support ‘Stepping Forward' to some of our service users. 

We have recently been invited by Dundee City Council to develop further services to support people with a learning disability moving from other care settings into their own homes.  We will work with our partners in health and social work over the next year to support these people with this transition.

Referrals:
The Dundee service works in partnership with Dundee City Council and Angus Council. At present access to our services are through direct referral and commissioning by Dundee City and Angus Council Social Work Departments.
We will, however, give advise on this process to anyone interested in our services.
